# Privacy
Your privacy matters most when the subject is your health. This page explains, in plain words, what we collect, why, where it goes and what you can ask us for.
Who we are
Bradford Naturopath is a naturopathic practice in Ontario. The practice is operated by Bradford Naturopath.
There are two kinds of information here, and they're handled differently:
- Contact details you send through this website, so we can set up your free call.
- Personal health information you share with the ND during your care.
What the free call form collects
The form on this site asks only what we need to call you back:
- your name;
- your phone number;
- an email address, if you choose to give one;
- which line sounds most like you (for example "I'm tired all the time."), so the ND knows where to start;
- how you'd like to talk: by phone, by video or in clinic;
- whether you'd like occasional news from the practice (a separate box, unticked unless you tick it).
The form never asks for your symptoms, conditions, medications, supplements, test results, date of birth or health card number. Please don't type health details into it. Those are for your conversation with the ND.
We use these details to contact you about your free call. If you give an email address, you may get an email confirming that your request arrived. If you ticked the news box, we may also send occasional news, and you can unsubscribe at any time.
The tick list on our pages
Some pages have a list you can tick, such as "Does any of this sound like you?". What you tick stays in your browser on your device and clears when you close the tab. It is never sent to us, never added to the form and never counted in our analytics.
Where your contact details go
When you press send, your browser sends your details directly to the customer relationship system we use to follow up on requests. They aren't sent by email.
Details you send through this website are stored and processed by KremCRM, the customer relationship system we use, on Microsoft Azure servers in Israel, outside Canada.
We use your contact details only to follow up on your request.
Your health information, protected under PHIPA
Health information you share during your care, such as your history, your lab results and your plan, is personal health information under Ontario's Personal Health Information Protection Act, 2004 (PHIPA).
Your health information is used to provide your care. It's shared only as PHIPA allows, for example with a lab that runs a test the ND requisitions, or with your family doctor when you give permission.
Your rights
Under PHIPA, you have the right to ask to see the personal health information we hold about you, and to ask us to correct it if it's wrong. You can also ask what contact details we hold about you, and to correct them. And you can ask us to delete the contact details you sent through this website.

If you're not satisfied with how we've handled your information or a request, you can complain to the Information and Privacy Commissioner of Ontario.
Keeping your information safe
We keep your information securely, with access limited to the people who need it to contact you or care for you.
How this website is measured
We use Google Analytics, which uses cookies, to understand how the site is used: which pages are viewed, which buttons are pressed, which line a visitor picks and whether a form was sent. It never receives your name, your contact details, anything you tick or any health information. You can block or clear cookies in your browser settings.
Changes to this page
If we change how we handle information, we'll update this page and the date at the top.
